Waiver & Release
I hereby declare that I am the legal owner of my animal(s) and have the authority to place them into respite care. My animal's care is not without risk. I authorize Murillo Mutts Respite Rescue to do whatever they deem necessary for the safety, health and well-being of my animal while under the care of Murillo Mutts Respite Refuge including seeking professional treatment for my animal. Every effort will be made to provide a safe environment for your animal. If your animal requires medical attention, we will contact you immediately. If we cannot reach you, we will transport your animal to the veterinarian on call. I waive any and all claims, actions or demands of any nature, foreseen or unforeseen, that I may have against Murillo Mutts Respite Rescue relating to the care, control, health and/or safety of my animal arising during pick-up, transportation, drop-off and stay at the facilities or at an approved foster home. I, the undersigned, agree to the above, accept full responsibility for the acts of my animal while being boarded and release Murillo Mutts Respite Refuge, and its owners, staff, and volunteers of all claims of liability. I acknowledge that I have carefully read all parts of the Agreement, including this Waiver and Release, understand its contents, and understand that this agreement includes an assumption of the risk of Murillo Mutts Respite Refuge negligence and liability. I acknowledge that Murillo Mutts Respite Refuge is materially relying on this waiver and is allowing my animal(s) to engage in play with other animals during their stay at Murillo Mutts Respite Refuge.
